We will be gathering data from the UNT Psycholkgy clinic assessment files that may possibly
include medical records. The c 'c staff will be providing specific files to us who meet the
requirements listed before (Le. receivedd an ADHD or LD diagnosis, completed the PAI). To
comply with HiPPA privacy rules we will not take any of the files Mt of the clinic and data will
be collected on the secured p rtale hard drives that will be kept in the PI's office.
Add itinally, we will not be collecting any information that could possibly include any of the
follcring: name and initials; street address, city, county. precinct, zip code, or equivalent
geondes; elerments cf dates (except year) directly related to an individual (date of birth,
admission date, discharge date, date of death}; elements of date including year for persons
90o r older telephone andlor fax n umtber; e-mail address; social security number medical
record or health plan identification number account number certificate and license number
vehicle identifier and serial n umber includi rig license plate number device identifier and serial
number web address (LFRL), internet IP address: bioruetric identifier including finger and
voice print, full face photographic image and comparable image; other unique identi-jying
number, characteristic, or code.83
