Description: During pre-restart testing the safety rod at position X26-YlO bound after being driven approximately two (2) feet out of the reactor. Subsequently, the rod was manually returned to it`s seated position. Description: In June 1990, during a three-site inspection of cylinders being used for long-term storage of solid depleted UF{sub 6}, two 14-ton steel cylinders at Portsmouth, Ohio, were discovered with holes in the barrel section of the cylinders. Description: As a result of the Concentrate Transfer System (CTS) tank ventilation system contamination event, a task team was formed to evaluate instrument loops associated with waste reduction equipment. During the event a conductivity probe designed to provide an alarm and initiate an interlock failed to respond to the presence of liquid.
