Infectious Diseases: Gaps Remain in Surveillance Capabilities of State and Local Agencies

Description: Testimony issued by the General Accounting Office with an abstract that begins "Recent challenges, such as the SARS outbreak and the anthrax incidents in the fall of 2001, have raised concerns about the nation's preparedness for a large-scale infectious disease outbreak or bioterrorism event. In order to be adequately prepared for such a major public health threat, state and local public health agencies need to have several basic capabilities, including disease surveillance systems, laboratory facilities, communication systems and a sufficient workforce. GAO was asked to examine the capacity of state and local public health agencies and hospitals to detect and report illnesses or conditions that may result from a large-scale infectious disease outbreak or bioterrorism event. This testimony is based largely on recent work, including a report on state and local preparedness for a bioterrorist attack; preliminary findings from current work on updates of bioterrorism preparedness at the state and local levels; and findings from a survey GAO conducted on hospital emergency department capacity and emergency preparedness."

Bayesian Probabilistic Reasoning Applied to Mathematical Epidemiology for Predictive Spatiotemporal Analysis of Infectious Diseases

Description: Abstract Probabilistic reasoning under uncertainty suits well to analysis of disease dynamics. The stochastic nature of disease progression is modeled by applying the principles of Bayesian learning. Bayesian learning predicts the disease progression, including prevalence and incidence, for a geographic region and demographic composition. Public health resources, prioritized by the order of risk levels of the population, will efficiently minimize the disease spread and curtail the epidemic at the earliest. A Bayesian network representing the outbreak of influenza and pneumonia in a geographic region is ported to a newer region with different demographic composition. Upon analysis for the newer region, the corresponding prevalence of influenza and pneumonia among the different demographic subgroups is inferred for the newer region. Bayesian reasoning coupled with disease timeline is used to reverse engineer an influenza outbreak for a given geographic and demographic setting. The temporal flow of the epidemic among the different sections of the population is analyzed to identify the corresponding risk levels. In comparison to spread vaccination, prioritizing the limited vaccination resources to the higher risk groups results in relatively lower influenza prevalence. HIV incidence in Texas from 1989-2002 is analyzed using demographic based epidemic curves. Dynamic Bayesian networks are integrated with probability distributions of HIV surveillance data coupled with the census population data to estimate the proportion of HIV incidence among the different demographic subgroups. Demographic based risk analysis lends to observation of varied spectrum of HIV risk among the different demographic subgroups. A methodology using hidden Markov models is introduced that enables to investigate the impact of social behavioral interactions in the incidence and prevalence of infectious diseases. The methodology is presented in the context of simulated disease outbreak data for influenza. Megadrought and Megadeath in 16th Century Mexico

Description: The native population collapse in 16th century Mexico was a demographic catastrophe with one of the highest death rates in history. Recently developed tree-ring evidence has allowed the levels of precipitation to be reconstructed for north central Mexico, adding to the growing body of epidemiologic evidence and indicating that the 1545 and 1576 epidemics of cocoliztli (Nahuatl for "pest") were indigenous hemorrhagic fevers transmitted by rodent hosts and aggravated by extreme drought conditions.

Towards Sustainable Global Health

Description: Global health has in recent years drawn increasing scientific, political and popular attention not only due to global epidemics themselves,but also because of the social activities and environmental conditions that shape health threats and influence those who are affected. The study dealswith the issue of 'Sustainable Global Health'which has evolved from the realization that there will be no alleviation of poverty without success in control of serious public health threats, no economic prosperity and sustainability without a healthy workforce, and no social stability and peace as long as people have to suffer from insufficient health services, from malnutrition, from HIV/AIDS pandemics, or from lack of safe water. The study addresses a broad range of issues related to human health at regional and global levels. It includes the theme of Technical and Vocational Education and Training (TVET) as a tool for the private sector to exercise responsibility and interest in using the workplaces as a route and as means for education, and for a wide participation of every citizen in securing his or her individual health and well-being. Highlighted throughout the study are integrated approaches towards sustainable health.These approaches shed light on both the importance of multilevel health governance and the understanding of human health as an issue of human security in responding to health threats. Furthermore,the study emphasizes the links between the phenomena of global environmental change, which often further increases pressure on health systems, and the crucial role urban areas play in this realm.
